How to start home care services

Your first step to starting home health care services is an in-person meeting with our home care coordinator to discuss your needs and/or those of your loved one, and to;
Assess Your Needs: Determine the type of home service you require. This could include medical services (such as skilled nursing, therapy, or home health aide assistance), personal care services (such as assistance with activities of daily living), household services (such as housekeeping or meal preparation), or other types of support. We also discuss any out-of-pocket cost if applicable. Typically, home health services are covered by insurance without additional cost to you.
Consult with your physician: If you need medical or therapeutic services, consult with your physician to discuss your needs, and obtain a prescription or referral for home services. Your physician can assess your condition, recommend appropriate services, and provide necessary documentation to initiate home care services.
Schedule an Assessment: Once we establish the need for care services and after consultation with your physician, we schedule a visit with intake RN for an assessment or evaluation. RN will assess your needs, conduct a thorough evaluation of your condition, and develop a personalized care plan tailored to your specific requirements.
Start Services: Once you have agreed to the care plan and completed any necessary paperwork, you can begin receiving home services according to the agreed-upon schedule. We will coordinate the delivery of services, assign caregivers as needed, and monitor your progress to ensure that your needs are being met effectively. We generally initiate services within a 48-hours after intake nurse visit.
Communicate and Provide Feedback: Maintain open communication with our office and caregivers. Provide feedback on the quality of care, any concerns or issues that arise, and any changes in your condition or needs. Collaboration and communication between you and our agency is essential for ensuring that you receive the best possible care and support at home.
